Prehabilitation in Physical Therapy - An IGNITE Summit Focus
As physical therapists begin to incorporate clinical practice into treating equestrian athletes, including both horses and riders, the concept of prehabilitation is reaching new territory.
Advances in veterinary regenerative medicine, such as the use of stem cells and platelet-rich plasma (PRP) to repair damaged or diseased tissues, has healthcare professionals asking how the concepts of prehabilitation in human clients might augment faster recovery and return to sport for high performance horses receiving similar medical treatments.

IGNITE EQUINE CLINCIANS REGISTRY RESEARCH PLAN:
A Prospective Longitudinal Study of Orthobiologics and Regenerative Medicine for the Treatment of Sport Injuries in Horses.
There is a paucity of systematically collected data available regarding the efficacy and safety of orthobiologics and regenerative products to treat active sport horses. Existing clinical research has its place in understanding product efficacy and safety, however, due to lack of funding and complexity of resource availability, many of these studies are being performed on horses who are not active in sport. Additionally, these studies suffer from relatively low case numbers, minimal comparative information, and lack of comparability between studies due to variability in outcome definitions. This leaves a gap between the findings of clinical research and practice. The horse Industry would benefit from the collection and reporting of well-defined, systematic, longitudinal, prospective data from a cross section of veterinary practices and equestrian disciplines using orthobiologics and regenerative therapies to treat suspensory injuries resulting in acute lameness and to treat clinically affected but not yet lame horses. This latter indication for treatment is unique to the equine industry and tracking and reporting outcomes from this approach will help establish data-driven evidence of effect.
With this initiative, we aim to:
1. Capture real insight into what is currently being used on equine athletes for different diagnoses.
2. Establish outcome parameters relevant to current clinical practice and the industry.
3. Offer impartial tracking of unexpected events following orthobiologics.
4. Initially: assess clinical efficacy of individual orthobiologics.
5. Eventually: assess comparative efficacy.
6. Provide a working platform for clinicians, to be modified as needed, to help make evidence-based decisions.
